The failure mode of AI governance is not permissiveness. It is a control regime so heavy that teams route around it.

Effective guardrails are specific, fast and explain themselves. A blocked action should tell the user exactly which policy applied and what the compliant path is.

We instrument every guardrail trigger. If a control fires constantly without changing behaviour, it is the wrong control.
